Estrogen Isn’t Just One Hormone – & Your Mouth Knows It

People often talk about estrogen as if it were a single hormone. It’s not.

Estrogen is actually a group of related hormones that work together in different ways throughout the body. It’s a distinction that matters, especially when we’re talking about oral health.

That’s because your saliva, gums, and other oral tissues don’t just respond to estrogen in general. They respond to specific forms of it.

Estradiol (E2) is the most potent of the bunch and the one most closely associated with reproductive and sexual health. Later in life, estrone (E1) becomes more dominant, particularly after menopause. Estriol (E3) is considered the mildest form and is known for supporting tissues such as those in the vaginal and urinary tracts.

But even that’s only part of the picture.

Estrogens don’t just circulate through the body unchanged. Once they’re produced, the body immediately starts changing them through a process known as estrogen metabolism, in which they’re broken down and converted into other compounds. It’s these metabolic pathways that help determine how estrogen interacts with tissues throughout the body and how strongly its signals are felt.

Some pathways are considered to be more protective. Others may be associated with greater inflammatory activity. In the realm of functional medicine, you’ll sometimes hear them described more simply as ‘’good” and “bad” estrogen pathways. The biology is actually a bit more nuanced than that, but the basic ideas – that how estrogen is metabolized matters – is an important one.

Shifts in estrogen metabolism can happen throughout life, but they often become especially noticeable during transitional times. Menopause is one of the most significant, when declining estradiol levels change the overall balance among the body’s estrogens and the way they’re processed.

But hormones don’t influence only reproductive tissues. Estrogen affects systems throughout the body, including the connective tissues, bones, and inflammatory signaling that help maintain oral health.

Dentists have long observed that hormonal shifts can influence the gums and bone that support the teeth. In fact, there are several points in life where the connection becomes more obvious. Hormonal changes can make the gums more reactive to bacterial plaque, which is why we see conditions like puberty gingivitis and pregnancy gingivitis. Both conditions tend to be self-resolving once the transitional period has passed, so long as good oral habits and care are consistent.

Menopause represents another hormonal transition. As estrogen levels shift and estradiol declines, the balance of inflammation and bone metabolism can change, too – including in the jawbone. For some women, this can mean increased gum sensitivity, gums that tend to bleed more, or a higher risk of gum disease if oral biofilm (plaque) isn’t carefully managed.

Seen in this light, oral health becomes part of a much broader physiological story – one that involves hormones, inflammation, bone metabolism, and the way the body processes estrogen itself.

And interestingly, many of the same lifestyle factors that support healthy estrogen metabolism also support oral health.

Nutrition is one example. Cruciferous vegetables such as broccoli, kale, and cauliflower contain compounds that help the body process estrogen through more favorable metabolic pathways. Such foods are a natural part of the kind of diet that helps support the immune balance that keeps the gums and other oral tissues healthy: whole foods, rich in fiber, rich in micronutrients.

The liver also plays a central role in estrogen metabolism, helping the body break down and eliminate hormone metabolites. Supporting liver function through adequate hydration, antioxidant-rich foods, and moderating alcohol intake can influence this process. These same habits also contribute to lower systemic inflammation, which benefits the oral tissues, too – especially the periodontal tissues that support the teeth.

Gut health is another important piece of the puzzle. Certain gut bacteria participate in the recycling and elimination of estrogen metabolites (waste products), helping regulate how these hormones circulate. At the same time, research continues to explore the connections between the gut microbiome, the oral microbiome, and systemic inflammation.

Regular movement and effective stress management round out the picture. Physical activity supports metabolic balance and bone health, while chronic stress can influence inflammatory signaling and hormone regulation throughout the body.

Taken together, these factors highlight an important point: Menopause isn’t simply about estrogen disappearing. It’s a transition in how the body produces, processes, and responds to hormones. Those changes can affect tissues throughout the body, the mouth included.
